Membership Directory Form

Keep your company in the minds of key mental health providers year-round! The Membership Directory, published each year, is used by ICCMHC members and other professional groups.

Deadline to submit contract and send artwork: MAY 21, 2010

Advertisement specifications:

• Ads are black and white only (other than outside cover)— all graphics included in artwork should be converted to grayscale.
• File format requirements: InDesign, QuarkXpress, Illustrator, Freehand and PDF. Additional fees will be applied for re-sizing or altering of ads.  
• Resolution must be a minimum of 300 dpi at full size.
• Do not submit Word, Powerpoint, or MS Publisher files.
